Please download the appropriate form, and send us the completed document via the contact form below, or you can simply email it to us at woolwich.optometry@gmail.com. Alternatively, the forms are printable and you can bring in your completed form to your appointment. The triage form is intended for anyone having an ocular emergency and should be sent to us right away, electronically (we will monitor these submissions M-F 8am-4pm). The intake forms are for new patients booking their initial appointment at Woolwich Optometry.
Optometry News!
We are open, during the pandemic. We’ve been seeing patients regularly, by appointment only. Over the past year, we have done our best at keeping everyone safe and comfortable in the office. Beyond the standard mask-wear recommendation, we clean every surface that patients touch, including all spectacle frames, between each and every patient visit. We have also modified our schedule so that physical distancing is the rule in the office.
We want all our patients to feel safe and comfortable when coming in for an eye exam. Please contact the office if you would like to book an appointment or if you have any questions. We are here help. This may include helping you to decide if now is the right time for an eye exam that you may have been putting-off due to the pandemic.
We look forward to seeing our patients, as well as their family and friends. And, we are accepting new patients!
